Can a blood test tell me I am in perimenopause?

Usually not by itself. Hormone levels vary so much from day to day during perimenopause that a single FSH or estradiol result can be misleading, which is why the transition is diagnosed mainly from your age and your cycle pattern. Bloodwork earns its place by ruling out the conditions that imitate perimenopause, thyroid disease and iron deficiency among them, and by establishing a baseline to measure against later.

Why one number is not enough

In early perimenopause your hormones are not declining in a straight line, they are fluctuating. Estradiol can read high one week and low the next. FSH does the same. Draw on the wrong day and the result argues against something you are clearly experiencing.

Where the numbers do carry weight

Later in the transition the picture steadies enough to be informative. An FSH above roughly 25 mIU/mL alongside a gap of 60 days or more without a period is one of the consensus markers of late perimenopause. That is a real signal. It is also a description of where you already are, not a prediction.

What the panel is actually for

This is the part that gets skipped. Several conditions produce the same fatigue, weight change, low mood and poor sleep, and they are all easy to find in blood.

  • Thyroid, which is the most common hormonal disorder in women after 40 and imitates perimenopause closely
  • Iron and ferritin, often low when periods have become heavier
  • Metabolic markers including glucose and A1c, which drift in this window
  • A hormone baseline to compare future results against, which only exists if you draw one now

The question worth asking instead

Not "do my labs prove I am perimenopausal" but "is there anything else here, and where do my numbers actually sit". That question a panel answers well.

How Regener8 approaches it

We draw 50+ markers across 9 categories at a LabCorp near you, and a board certified clinician reads them with you on a 30 minute visit. Your cycle history and how you feel are part of that conversation, not an afterthought to the printout.
