Regener8 Health & Wellness, LLC ("Regener8," "we," "our," or "us") respects your privacy. This Privacy Policy explains what information we collect, how we use it, when we share it, and the choices you have. It applies to regener8now.com and its subdomains, our patient portal, our emails and text messages, and any other service that links to this Policy (together, the "Services").
Some of the information we collect is Protected Health Information (PHI) under the federal Health Insurance Portability and Accountability Act (HIPAA). PHI is governed by our Notice of Privacy Practices, which is provided to you when you become a patient. Where this Privacy Policy and the Notice conflict on PHI, the Notice controls.
1. Who we are
Regener8 Health & Wellness, LLC, 2901 NW Commerce Park Dr, Suite 1, Boynton Beach, FL 33425. Contact: dt@regener8now.com.
2. Information we collect
Information you give us.
- Identifiers: name, date of birth, postal address, email, phone number.
- Account credentials: username, password (stored hashed), authentication factors.
- Health and wellness information (in the intake and patient record only): medical history, medications, allergies, lab results, treatment notes, telehealth visit summaries, secure messages.
- Payment information: billing name and address and the last four digits of a payment card. Full card numbers are handled by our third-party payment processor and are not stored on our systems.
- Communications: emails, text messages, form submissions, voicemails.
- Marketing preferences: opt-ins, opt-outs, and interests you tell us about.
Information collected automatically. When you use the Services, our hosting and infrastructure providers may collect your IP address, device and browser type, the pages you view, the link that referred you, and an approximate location inferred from your IP address. See our Cookie Policy for what this website does and does not set on your device.
Information from third parties. Our partner laboratory, LabCorp, sends us your test results when you authorize it. Our licensed partner pharmacies send us fulfillment and shipping status.
3. How we use information
- Provide, operate, and improve the Services.
- Schedule visits, coordinate lab work, fill prescriptions, and communicate results.
- Process payments and prevent fraud.
- Send appointment reminders and shipping notifications.
- Send marketing communications you have opted in to receive. You can opt out at any time.
- Comply with legal obligations and enforce our Terms.
- Understand and improve performance, security, and the experience of using the Services.
We do not use PHI for marketing without your written authorization, except as HIPAA permits.
4. How we share information
We share information only as described here. We never sell your personal information or PHI.
- Treatment, payment, and healthcare operations: with our licensed clinicians, LabCorp, our partner pharmacies, and our care coordinators, as described in the Notice of Privacy Practices.
- Service providers under contract that restricts how they may use the data and, where PHI is involved, signed Business Associate Agreements. Today these include our patient record and scheduling system (PracticeQ, also known as IntakeQ), our customer relationship and messaging platform (GoHighLevel, which receives contact details and marketing consent but not clinical records), our payment processor, our email and document providers, and our hosting and infrastructure providers.
- Legal and safety disclosures: when required by law, subpoena, or court order, or to protect rights, safety, or property.
- Business transfers: in connection with a merger, acquisition, financing, or sale of assets. We will notify you of any change in ownership or in how your information is used.
- With your consent: for any other disclosure you authorize.
5. Text messages and email
If you provide your phone number and opt in, we send transactional and marketing text messages governed by our SMS Terms. Reply STOP to cancel and HELP for help. Message and data rates may apply. You can unsubscribe from marketing email using the link at the bottom of any marketing message. Transactional and clinical emails, such as appointment confirmations and prescription updates, continue while you are an active patient.
6. Your privacy rights
Depending on where you live, you may have the right to access the personal information we hold about you, correct it, delete it (subject to legal and clinical record retention requirements), restrict or object to certain processing, receive a portable copy of certain information, withdraw consent for marketing, and file a complaint with a regulator, including the U.S. Department of Health and Human Services for HIPAA matters.
California residents: you may also request the categories and specific pieces of personal information we have collected, and you will not be treated differently for exercising your rights. We do not sell personal information.
To exercise any right, email dt@regener8now.com. We may need to verify your identity first. Patients can request copies of their medical records through their care team.
7. Data retention
We keep information for as long as needed to provide the Services and for legitimate business or legal reasons. Medical records are retained for at least seven years from the date of last service, or longer where Florida law requires. Marketing data is kept until you opt out, plus a reasonable period on a suppression list so we do not contact you again.
8. Security
We use administrative, physical, and technical safeguards aligned with the HIPAA Security Rule: encryption in transit and at rest, role-based access, multi-factor authentication for staff, audit logging, vendor agreements, and secure disposal. No system is perfectly secure. If a breach occurs we will notify affected individuals and regulators as the law requires.
9. Children
The Services are for adults. We do not knowingly collect personal information from anyone under 18, and we will delete it if we learn we have.
10. Users outside the United States
The Services are operated from the United States and are offered to U.S. residents. If you access them from elsewhere, your information is transferred to and processed in the United States.
11. Third-party links
The Services may link to sites we do not control. We are not responsible for their privacy practices.
12. Changes to this Policy
We may update this Policy. The date at the top reflects the latest change. If a change is material, we will notify you by email or in the patient portal. Continued use of the Services after the updated Policy takes effect means you accept it.
