You cannot know whether hormone therapy is right for you at 40 from symptoms alone. Low energy, poor sleep, weaker workouts and falling drive have several causes that feel identical, and some of them are not hormonal at all. The reliable starting point is a blood panel that covers testosterone, thyroid, metabolic and blood markers together, read by a clinician who can tell which one is driving what.
Why the symptoms cannot answer the question
Fatigue, brain fog, longer recovery and lower drive are real, and they are also the symptoms of an underactive thyroid, iron deficiency, poor sleep, rising blood sugar and ordinary stress. Two men with the same complaints can have completely different bloodwork. A questionnaire can flag that something is off. It cannot tell you what.
What a panel shows that a quiz cannot
A comprehensive draw puts the possible causes side by side so they can be told apart. The markers that matter most for this question:
- Total and free testosterone, with SHBG. Total testosterone alone can mislead, because SHBG decides how much of it your body can actually use.
- Estradiol. Men make it from testosterone, and the balance between the two matters more than either number alone.
- TSH, Free T4 and Free T3. An underactive thyroid produces almost exactly the low testosterone symptom list.
- Glucose and hemoglobin A1c. Rising blood sugar drains energy long before it becomes a diagnosis.
- Ferritin and a complete blood count. Low iron and low hemoglobin are a common, easily missed cause of tiredness.
What changes after 35, and what does not
Testosterone tends to drift down with age, and the pace varies widely from one man to the next. Some notice nothing. Others feel a clear shift in energy, recovery, mood or libido. Neither is a diagnosis. Levels also move with sleep, body composition and illness, which is why one low reading is a reason to look closer, not a verdict.
What a good answer looks like
Sometimes the panel supports hormone therapy. Often it points somewhere else first: thyroid, iron, sleep, or blood sugar. A clinician who sees the whole picture can say which, and can say when the honest answer is that your numbers do not support treatment yet.
How Regener8 approaches it
Every hormone conversation here starts with the panel: 50+ markers across 9 categories, drawn at a LabCorp near you. A board certified clinician goes through the results with you on a 30 minute video visit, and only then is a plan on the table. If your labs do not support therapy, you will hear that. If they do, you retest every six months so the plan follows your numbers.
