It depends on your age, how long since your final period, and which formulation. Current professional guidance supports hormone therapy for symptomatic women under 60 or within 10 years of menopause who have no contraindications. Breast cancer risk is real but formulation specific: combined estrogen plus progestogen carries more risk than estrogen alone, risk rises with duration of use, and vaginal estrogen has not shown the same association. This is a conversation with a clinician who has read your history, not a decision to make from an article.
What happened in 2002, and what got lost
The Women's Health Initiative results were reported in a way that made hormone therapy sound uniformly dangerous, and prescribing collapsed. The trial population was older than the women who typically want treatment, and the results were not separated by age or formulation in the coverage. Both of those turn out to matter enormously.
Timing changes the answer
Professional guidance now supports treatment for symptomatic women who are under 60 or within 10 years of their final period and have no contraindications. An age stratified reanalysis of the WHI found that in women aged 50 to 59 with moderate to severe hot flashes, both regimens studied relieved symptoms without raising cardiovascular risk. The same analysis advises caution from 60 onward.
Formulation changes it too
A meta analysis of the worldwide epidemiological evidence found excess breast cancer risk with every type of therapy except vaginal estrogen, rising with duration and greater for estrogen plus progestogen than for estrogen alone. In the WHI itself, conjugated estrogen alone did not increase breast cancer at 20 year follow up, while estrogen combined with medroxyprogesterone acetate did.
- Combined therapy carries more risk than estrogen alone, and the difference is not small
- Risk rises with duration, so how long is part of the decision, not a detail
- Vaginal estrogen has not shown the same association
- Whether you still have a uterus determines whether a progestogen is needed at all
What this does not mean
It does not mean hormone therapy is risk free, and it does not mean everyone should be on it. It means a blanket yes or no is the wrong shape of answer. Your family history, your cardiovascular and clotting risk, how far you are from your final period and how much your symptoms are costing you all belong in the same conversation.
How Regener8 approaches it
Nothing is prescribed off a symptom questionnaire. A board certified clinician reviews your full panel and your history with you on a 30 minute visit, and goes through the specific risks that apply to you rather than the general ones. If hormone therapy is not appropriate, that is a legitimate outcome of the visit.
