Low libido after 40: what is hormonal and what is not.

Falling desire after 40 can be hormonal, but it is rarely only hormonal. Clinical guidance for treating low desire in women is explicit that relationship factors, mood, medication side effects, sleep and pain during sex should be assessed before hormonal treatment is considered. Bloodwork is genuinely useful here for finding the physical contributors, thyroid disease, iron deficiency and the sex hormones among them, but a blood testosterone level is not used to diagnose low desire.

Desire has more than one input

The clinical guidance is unusually clear on this point. Systemic testosterone is recommended only for low desire that is not primarily explained by modifiable factors, and it names those factors: relationship difficulties, mental health, medication. That is not a soft caveat. It is the order of operations.

What is physical and worth measuring

Several physical contributors are common, easy to find and easy to miss.

  • Falling estradiol, which changes vaginal tissue and can make sex uncomfortable, and discomfort reduces desire on its own
  • Thyroid dysfunction, common after 40 and a direct cause of low energy and low libido
  • Iron deficiency, especially where periods have become heavier
  • Medications, with SSRIs and some blood pressure drugs among the usual contributors

Where testosterone fits

For postmenopausal women with hypoactive sexual desire disorder, an international consensus supports testosterone, and reviews show a moderate benefit for desire, arousal and pleasure. Two things are worth knowing: there is no FDA approved female testosterone product in the US, so prescribing is off label, and the diagnosis is not made from a blood level.

The part nobody says

Low desire is only a problem worth treating if it is a problem to you. The clinical definition includes distress for a reason. A change that does not bother you is not a condition.

How Regener8 approaches it

You share your history privately through a clinical intake, draw a panel that covers the sex hormones, thyroid, iron and metabolic markers, and go through all of it with a board certified clinician on a private video visit. Nothing is prescribed from a questionnaire, and nothing is assumed about what you want.
