Perimenopause is the transition leading up to your final period, and for most women it begins somewhere in the forties and lasts several years. FSH starts climbing roughly six years before the final period, while estradiol stays high and erratic until much later. That mismatch is why symptoms can start long before anything looks obviously wrong, and why a single blood draw rarely settles the question on its own.
It starts earlier than most people are told
Menopause is a single day: twelve months after your last period. Perimenopause is everything before it, and it is the part that actually affects how you feel. For most women it opens in the early to mid forties and runs for years, not months.
What moves first
The order matters, because it explains why you can feel different while a standard panel looks unremarkable. FSH begins rising about six years before the final period. Estradiol does not simply fall over that stretch. It swings, and through early perimenopause it averages roughly 20 to 30 percent higher than in premenopausal women. Progesterone behaves differently again: it holds relatively steady early on and declines later in the transition.
- FSH climbs first, years ahead of anything else
- Estradiol swings rather than dropping, and can run higher than usual before it falls
- Progesterone declines late, which is often when sleep and mood change
Why the symptoms feel unrelated to each other
Sleep that breaks at 3am, a shorter fuse, weight settling around the middle, cycles that shorten then lengthen, joints that ache without a reason. These do not present as one condition, so they tend to get treated as five separate problems by five different people.
When testing is worth doing
Because estradiol and FSH move so much from week to week in early perimenopause, one draw is a snapshot, not a verdict. What bloodwork does well is rule things in and out: thyroid, iron, metabolic markers and the sex hormones together, read by someone who knows what stage you are likely in. That is a different question from proving you are perimenopausal, and it is the more useful one.
How Regener8 approaches it
Regener8 is a telehealth wellness clinic for adults 30 and older. We draw a full hormone, thyroid and metabolic panel, and a board certified clinician goes through it with you on a 30 minute visit before anything is recommended. If your numbers do not support treatment, we will tell you that.
