Testosterone helps men build and keep muscle and limits fat storage, especially around the middle. As levels drift down after 40, the balance shifts: harder to hold muscle, easier to gain fat, and the added fat pushes testosterone lower still. The Endocrine Society's guideline notes that treating a real deficiency increases lean mass and reduces fat, but it also sets a high bar for what counts as a deficiency: consistent low readings plus symptoms, not one number.
What testosterone does to body composition
Testosterone signals muscle to grow and repair, and it influences where fat is stored. Men with healthy levels hold muscle more easily and carry less fat around the organs. When the level falls, the same training and the same food produce a different body. That is the change many men notice in their forties and blame on age or discipline.
The loop with belly fat
Visceral fat converts testosterone into estradiol and produces signals that suppress its production. So more belly fat means less testosterone, and less testosterone means more belly fat. Researchers now describe low testosterone in many men as a feature of that fat rather than an isolated hormone problem, which is why the two are read together.
What the evidence says about treating it
In men with a genuine deficiency, the Endocrine Society's guideline reports that testosterone therapy increases lean mass and strength and reduces total and abdominal fat. A trial in obese men on a calorie restricted diet found treatment helped them keep lean mass while losing fat. The gains depend on the dose and on how low the starting point was, and they are not a substitute for the diet and training that made them possible.
What a clinician looks at first
- Total and free testosterone, on a morning draw, and read against your symptoms.
- Estradiol and SHBG, which change how much testosterone is actually available.
- A1c, glucose, triglycerides and HDL, because insulin sensitivity sits underneath all of this.
- PSA and hematocrit, which are checked before and during any therapy.
How Regener8 approaches it
The panel comes first, drawn at any LabCorp, and a board certified clinician goes through it with you on a 30 minute video visit. If a plan is right for you it is built from those numbers, shipped to your door, and retested every six months so it keeps matching where you are.
